The purpose of this assignment is to critically review the process of care planning. This shall be done by examining the topic of care planning, focusing on the chosen client and researching the relevant evidence based practice. For the purposes of confidentiality all persons and places have been changed in accordance with the Nursing and Midwifery Council (2002)The client focussed on for this assignment shall be referred to as "Gordon". He was a 76 years old gentleman who had been transferred to the ward for rehabilitation following a right dynamic hip screw operation (DHS) correcting a fractured hip following a fall at home. Gordon's ongoing health need was that he suffered from Parkinson's which was controlled with drug therapy. Gordon displayed several of the classic symptoms for Parkinson's, (Larsen 1997) slow, stiff, shaky movements, which he had difficulty starting. He Stooped (hunching forward) shuffling when he walked, along with the DHS operation this all limited Gordon's mobility.

GordonGordon needed a care plan that would facilitate his recovery from the operation but also incorporate the ongoing health needs of his Parkinson's. Another major contributing factor was Gordon's confidence, as a fall had caused his fracture, he was now afraid to mobilize for fear of falling again, this fear was limiting his abilities to return to normal mobility.
